Service Description

Please read thoroughly before booking: You will be sent an intake form after you book the appointment. Please fill this form out in detail and submit it one week before your appointment. This will give me enough time to prepare for your appointment. Not sending this form back in time could result in having to reschedule your appointment and possibly incurring a $25 rescheduling fee. During this consultation we will review timeline of the health concern(s), symptoms, family history, labs if also summited one week prior to the appointment. After the consultation I will pull together our findings and a "game plan" going forward to address your health concerns and bring you relief and answers. Initial consultations can run up to 2 hours. Recovery does not happen overnight. This initial consultation will set you on the right path. Many people need 3-6 months of follow ups, some more, some less. I create a unique plan per individual. If additional labs are needed during this time, it's only $50 fee to view those labs with you. This does not cover the cost of the labs. I recommend you work with your doctor to run the labs when able to so insurance reduces the out-of-pocket cost.
